Hazard Alert No 75 - Lifts - Inspection and Testing was issued in September 2005 to advise lift manufacturers, installers, maintenance service providers and lift owners of their occupational health and safety obligations to carry out appropriate inspections, testing and regular maintenance on lifts.

Hazard Alert No 35 - Equipment failure due to lack of maintenance was issued in July 2001.

An employee was loading cardboard into a bale press when the crusher plate became detached from the ram rod. The crusher plate fell on to the employee's arm, fortunately in this instance resulting in only minor injuries.

These injuries could have been much more severe.

Hazard Alert No 112 - Blade Failure of a wood chipper was issued in October 2012.

A wood chipper machine was in use when a sudden failure resulted in a cutting blade shattering and large fragments were ejected through the solid metal guard.

The failure had the potential to cause serious injury to the woodchipper operator or other workers in the vicinity.
