14/2/92 T O'CONNOR & SONS PTY LTD (TOC PTY LTD), was convicted and fined $30,000 in respect of one breach of Section 19(1). Convictions without penalty were recorded for six other breaches of the same section. Counsel fees of $200 plus $202 costs were also imposed. Seven workers were working on an unprotected platform some 70 feet high. One of the workers fell to his death on 27 July 1990.

19/2/92 DAVID JOHN BYRNE (A FOREMAN), was convicted and fined $350 plus $93 costs for a breach of Section 21(1)(b). He interfered with solenoid switches while a worker was performing maintenance work on a machine, causing the worker to suffer amputation injuries to several fingers on 2 April 1991.

19/2/92 PIRELLI CABLES AUSTRALIA LTD, was convicted and fined $3,000 plus $90 costs for a breach of Section 19(1). Worker had her fingers trapped in a Moulding Machine on 13 October 1990.

19/2/92 FAST FORMWORK INDUSTRIES (SA) PTY LTD, was convicted and fined $4,500 plus $119 costs for a breach of Section 19(1). Failure to observe the provisions of an approved Code of Practice for the safe removal of asbestos on 6 February 1991.

18/3/92 SCREENINGS PTY LTD, was convicted and fined $6,000 plus $93 costs for a breach of Section 19(1). Worker suffered fractured wrists and abrasions when he fell on 1 August 1991.

18/3/92 BRICE MANUFACTURING PTY LTD, was convicted and fined $7,000 plus $93 costs for a breach of Section 19(1). Worker received crush injuries on an ineffectively guarded Reimann Combined Guillotine/Rolling Machine on 8 October 1991.

18/3/92 GEORGE KALDIS (A SUPERVISOR), was convicted and fined a total of $600 in respect of 3 breaches of Section 21(1)(b). A conviction but no penalty was recorded in respect of 2 other breaches of Section 21(1)(b). Costs totalling $186 were imposed. Five workers suffered burns when they were engulfed by steam while performing demolition work at the BHP steelworks at Whyalla on 30 April 1991.

9/4/92 DEREK JOHN FOSTER (A PRINTER), was convicted and fined $250 plus counsel fee of $250 plus $73 costs for a breach of Section 21(1)(b). An apprentice suffered a fracture to his right arm plus other injuries when he became trapped in a printing press on 29 April 1991.

10/4/92 COOK CONSTRUCTIONS PTY LTD, was convicted and fined $35,000 plus $173 costs for one breach of Section 19(1). Convictions without penalty were recorded for four other breaches of the same section. 15/4/92 ASM AUSTRALIA PTY LTD, was convicted and fined $1,000 plus $93 costs for a breach of Section 19(1). Worker suffered injuries to her fingers while working on a frypan base trimming machine on 28 August 1991.

15/4/92 THE VELLUM BINDERY PTY LTD, was convicted and fined $1,500 plus $93 costs for a breach of Section 19(1). A worker suffered injuries when her long hair became trapped and wrapped around a shaft on a machine on 20 August 1991.

20/5/92 C P DETMOLD PTY LTD, was convicted and fined $15,000 plus $93 costs for a breach of Section 19(1). A worker suffered lacerations and crush injuries to his right hand when it became trapped in a Post Folding Box Machine on 28 June 1991.

3/6/92 TOMO FIRLI (A BUILDER), was convicted and fined $400 plus Counsel fee of $600 plus $450 witness fees plus $73 costs for a breach of Section 19(1). A worker fell from an unsecured box on the tines of a fork-lift on 15 November 1991.

12/6/92 ROBIN ANTHONY SHARP (A WORK'S MANAGER), was convicted and fined a total of $1,100 plus counsel fee of $846 plus $223 costs for eight breaches of Section 21(1)(b). Seven workers were working on an unprotected platform some 70 feet high on 26 & 27 July 1990.

24/6/92 ANI CORPORATION LTD, was convicted and fined $15,000 plus $93 costs for a breach of Section 19(1). A worker suffered injuries when a moulding box being transported on a front end loader fell and hit the operator on 14 November 1991.

25/6/92 SCHLUMBERGER SEACO INC, was convicted and fined $3,500 plus counsel fee of $160 plus $73 costs for a breach of Section 24(2). A contractors employee suffered severe injuries when working on a oil drilling rig on 27 January 1990.

25/6/92 EVANS DEAKIN INDUSTRIES LTD, was convicted and fined $8,000 plus counsel fee of $1,100 plus $73 costs for a breach of Section 19(1). A worker fell from the tines of a fork-lift on 26 September 1990.

1/7/92 TERRY JOHN SWANTON, (A SAFETY CO-ORDINATOR), was convicted and fined a total of $1,100 plus $166 costs for five breaches of Section 21(1)(b). Five workers suffered burns when they were engulfed by steam while performing demolition work at the BHP steelworks at Whyalla on 30 April 1991.

14/7/92 WILLIAM FREDERICK PAUES (A WORK'S MANAGER),was convicted and fined a total of $880 plus counsel fee of $160 plus $193 costs for seven breaches of Section 21(1)(b). Seven workers were working on an unprotected platform some 70 feet high on 23 July 1990.

27/8/92 DAVID VALOVE (A MANAGER), was convicted and fined a total of $650 plus $151 costs for three breaches of Section 38(8). Hindered an inspector, and failed without lawful excuse to answer truthfully questions put to him by an Inspector on 2 April 1992.

28/8/92 APCEL PTY LTD, was convicted and fined $11,000 plus a counsel fee of $300 plus witnesses expenses of $244 plus $73 costs for a breach of Section 19(1). A worker suffered injuries to fingers on his left hand when they became trapped in a rewinder machine on 15 June 1991.

2/9/92 ATCO-APM DRILLING PTY LTD, was convicted and fined $500 plus $600 counsel fee plus $200 disbursement fee plus $73 costs for a breach of Section 19(1). A worker suffered severe injuries when working on an oil drilling rig on 27 January 1990. Prosecution appealed against manifestly inadequate penalty. (see 11/11/92).

23/9/92 UNIVERSAL INDUSTRIES LTD, was convicted and fined $10,000 plus $98 costs for a breach of Section 19(1). An apprentice suffered severe burns when cleaning an air receiver as directed on 18 April 1991.

23/9/92 KENNEDY CLEANING SERVICES (AUST.) PTY LTD, was convicted and fined $500 plus $98 costs for a breach of Section 19(1). A worker suffered severe bruising to her upper thigh at the premises of GMH while performing cleaning duties. An automatic machine had not been isolated on 23 February 1992.

11/11/92 ATCO-APM DRILLING PTY LTD, (see 2/9/92). The Court allowed the appeal against a manifestly inadequate penalty and quashed the penalty of $500.00 and in lieu thereof imposed a fine of $3,500.00.

18/11/92 BORAL HOLLOSTONE MASONRY (SA) PTY LTD, was convicted and fined $11,000 plus $109 costs for a breach of Section 19(1). A worker suffered amputation injuries to two fingers while performing his duties involving fork-lift operations on 5 March 1992.

2/12/92 CSR LIMITED, trading as "CSR WOOD PANEL", was convicted and fined $7,000 plus a counsel fee of $2,500 plus $73 costs for a breach of Section 19(1). A worker suffered a back strain while lifting a 50kg bag on 16 August 1991. Defendant appealed against conviction and penalty. (see 18/5/94).

3/12/92 ACE HOT WATER PTY LTD, was convicted and fined $3,500 plus a counsel fee of $750 plus $84 costs for a breach of Section 19(1). A worker received injuries while operating an ineffectively guarded machine on 21 November 1991.
