17/2/93 CASTALLOY MANUFACTURING PTY LTD, was convicted and fined $4,000 plus $114 costs for a breach of Section 19(1). A worker suffered serious injuries when he fell through the eaves of a building while working as directed on 3 December 1991.
17/2/93 SEAS SAPFOR LTD, was convicted and fined $9,000 plus $99 costs for a breach of Section 19(1). A worker received serious injuries while operating an ineffectively guarded Weinig 22B Moulder Machine on 9 June 1992. They were also convicted and fined $3,000 plus $99 costs for a breach of Section 19(3). Failure to ensure that a supervisor was instructed and trained as necessary on 9 June 1992.
18/3/93 MOBIL OIL AUSTRALIA LTD, being the deemed employer pursuant to Section 4(2) was convicted and fined $23,000 for a breach of Section 19(1). They were also convicted and fined $7,000 for a second breach of Section 19(1). Two maintenance workers suffered severe burns after being drenched with hot oil on 24 January 1990. One of the workers died 3 months later as a result of the burns.

19/5/93 WHITES MACROB FURNITURE PTY LTD, was convicted and fined $500 plus $114 costs for a breach of Section 19(1). A worker received injuries to his right hand when it became trapped in plant not in a safe condition on 7 August 1992.

16/7/93 GRIFFIN PRESS LIMITED, was convicted and fined $7,500 plus $1,510 counsel fees plus $90 witness fees plus $84 costs for a breach of Section 19(1). A worker suffered injuries to his left hand when it became trapped in a printing press on 4 October 1991.
21/7/93 ELVJON NOMINEES PTY LTD, was convicted and fined $4,500 plus $114 costs for a breach of Section 19(1). A worker suffered an amputation injury to the first joint of his right index finger while operating a 35 ton inclinable power press that was in an unsafe condition on 25 February 1993.
21/7/93 CASELEA INDUSTRIES PTY LTD, was convicted and fined $4,000 plus $114 costs for a breach of Section 19(1). A worker received severe injuries to three fingers of his left hand while operating a 'hand held' router that had been fixed to the underside of a work bench on 29 October 1992.
21/7/93 JAMES HARDIE IRRIGATION PTY LTD, was convicted and fined $1,000 plus $114 costs for a breach of Section 19(1). A worker received head injuries when her long unrestrained hair became entangled in a pedestal drill on 23 September 1992.
10/9/93 SOFTWOOD HOLDINGS LTD, was convicted and fined $38,000 plus $4,200 counsel fees plus $84 costs for a breach of Section 19(1). A worker died after a stack of timber collapsed and fell on him on 19 March 1992. Defendant has appealed against the conviction and penalty. (See 16/2/95,
15/10/93 KARINA FISHERIES PTY LTD, was convicted and fined a total of $30,000 plus $1,380 counsel fee plus $106 costs for two breaches of Section 19(1). A worker died on a fishing vessel at sea as a result of failure to provide a safe system of work on 5 May 1992.
