13/7/01 SA Sawmilling Pty Ltd: Were convicted and find $33,500 for breaching S19 for failing to provide a safe systems of work, S20 for failing to maintain policies and procedures and S61 for failure to nominate a Responsible Office. On 22 November 1999 an employee was killed after falling onto a moving conveyor belt while sweeping bark chips off a section of the sawmill equipment called a "jack ladder" at the sawmill at Meadows.
25/7/01 Holstens Pty Ltd: Were convicted and fined $150 for breaching S 20 having failed to prepare policies and procedures and failed to undertake a risk assesment of plant (R 3.3.1).
